Q: What is considered “customer information” and how do we collect it?
Customer information means non-public, personally identifiable information about a consumer or a consumer’s current or former relationship with us.
We collect and use only the personal information about you that is necessary to provide superior customer service, maintain and administer your account and provide financial services that are of interest to you. Generally, the information we gather is provided directly by you. Additional information may be gathered from outside sources such as public records, market research and credit reports. The type of information we collect and use is:
• Application Information - Information we receive
from you from applications or other forms such
as your name, address, Social Security Number,
assets or income;
• Transaction Information – Information about
your transactions with us such as your account
balance or payment history;
• Third-Party Information – Information such
as household demographics obtained from specialized
marketing information firms; and
• Consumer Reporting Information - Information
we receive from consumer reporting agencies
and others such as your creditworthiness and
credit history.
Q: Do you sell customer information to third
parties?
We do not sell customer information to any third
parties; therefore, you do not need to take
any action to prevent us from selling your information.
We maintain physical, electronic and procedural safeguards to guard your non-public personal information. We allow employees to access information about you when that access is necessary to maintain your accounts, service your needs or perform other required activities. Additionally, we continually educate our employees about the importance of confidentiality and customer privacy. Our employees are governed by a Code of Ethics that includes the responsibility to protect the confidentiality of each customer’s personal information. Even if you are no longer our customer, we will continue to treat your non-public personal information with the same care as if you were still a customer.
Q: With whom do you share my information?
We limit who receives customer information and what type of information is shared. The following are circumstances in which your information may be shared.
• Sharing Information with Outside Service Providers
– We may disclose customer information to companies
to provide products and services on our behalf.
• Information Disclosed in Other Circumstances
– We may disclose information to credit bureaus
or other similar reporting agencies. If you
are involved in litigation with a third party,
we may be ordered to provide information to
a court or other litigant. In these circumstances,
we will only disclose the specific information
required by law in connection with the subpoena
or court order.
• Medical Information – We occasionally receive
medical information about a customer, for example,
if a customer applies for insurance from us.
We will not share medical or health information
except as necessary to maintain accounts, process
transactions or provide services the customer
has requested or initiated.
Q: What if I discover a discrepancy in the information you have shared?
We continually strive to ensure that the information we maintain about you is complete and accurate. You have the right to notify us if you believe we have reported inaccurate information about you or your account to any consumer reporting agency. Notices should be sent in writing and include your complete name, Social Security Number, current address, telephone number, account number, type of account, specific item in dispute and the reason why you believe the information reported is in error. Q: I am not comfortable with you sharing my
information with third parties. Can I limit
or restrict this exchange of information?
By sharing information with third parties, it is our intention to provide you with a full array of financial services specifically tailored to meet your needs. By sharing information with third parties, we may be able to introduce you to new financial services. However, if you do not want us to share non-public information about you with third parties, you may opt out of those disclosures by calling ProvInsure, Inc. at 407-370-0776. You may contact us at any time to change your election.
Q: Can I restrict telephone, direct mail and e-mail solicitations?
We understand that you may wish to limit how you receive marketing offers. You may request to be excluded from telephone, direct mail and e-mail solicitations from us and from third party providers working with us. If you do not wish to receive such solicitations, inform us of your desire to be excluded by calling ProvInsure, Inc. at 407-370-0776. You may contact us at any time to change your election.
Q: I have a joint account with another individual.
Can I limit or restrict the sharing of information
for that individual as well?
Each owner of a joint account must elect to
block the sharing of their information separately,
although both parties may notify us in the same
communication.
If you have previously opted out, you do not
need to do so again. Please call ProvInsure
at 407-370-0776 to opt out of information disclosure.
*Please note that you may still receive mailings
if your information was permissibly shared prior
to processing your opt-out request or if the
mailings can be provided without violating your
opt-out request.
We will provide our Privacy Notice annually,
as long as you maintain an ongoing relationship
with us. We reserve the right to change this
Privacy Notice at any time. You will be advised
of any changes.
